Total iron-binding capacity (TIBC) is the measurement of the maximum concentration of iron that transferrin can bind. When iron stores are depleted, transferrin levels increase in blood. Only one-third of transferrin is saturated with iron; therefore, the transferrin present in serum has extra binding capacity. This is unsaturated iron-binding capacity (UIBC), i.e., the measurement of the number of sites on transferrin that are not already carrying iron. TIBC = UIBC + Serum Iron.
